Community Connectors Program (CCP)
The Migrant Information Centre (Eastern Melbourne) (MIC) has received funding for the Community Connectors Program (CCP) to support people with disabilities from Culturally and Linguistically Diverse (CALD) backgrounds to understand and engage with the National Disability Insurance Scheme (NDIS).
The program has been established in recognition that people with disability from CALD communities face additional barriers to accessing the NDIS. The main purpose of the program is to reduce these barriers and support participants to achieve their goals.
Community Connectors will:
- Assist individuals and their families/carers/representatives to complete an Access Request.
- Assist individuals and their families/carers/representatives to attend appointments to gather functional assessments and other evidence that may support their application.
- Attend NDIS pre planning meetings, planning meetings and request a review of an access decision (if required).
- Connect potential clients with NDIS Partners in the Community (PITC) such as LAC providers.
- Activate and implement plans by supporting clients to engage with service providers and other supports.
Eligibility:
- Younger People with a disability from CALD backgrounds (7 years to 64 years of age).
- Live within the Eastern Metropolitan Region of Melbourne (EMR). The EMR includes local government areas: Boroondara, Knox, Manningham, Maroondah, Monash, Whitehorse and Yarra Ranges.
- Need help to access NDIS and other related services.
