NEPHU Population Health Catchment Plan Part 1 (2023 - 2025)
NEPHU is very excited to release their Population Health Catchment Plan.
The NEPHU Population Health Catchment Plan aims to guide collaborative efforts and prioritise health promotion and prevention initiatives to support communities in living healthier lives – now and into the future.
Developed in partnership with key stakeholders and informed by a multi-stage planning process – which included a review of the prevention landscape, consultation with over 40 stakeholder groups, and the development of a population health profile – the plan identifies two priority areas for collective action:
- Increasing health eating
- Improving sexual and reproductive health.
Guided by the principles of collaboration, equity, and empowerment, and informed by various approaches – such as collective impact, evidence-informed strategies, and intersectionality – activity in the plan is structured around three domains:
- Chronic disease primary and secondary prevention activity
- Health protection-related prevention activity
- Population health service system support and coordination.
The plan is strongly aligned with state-wide and sector-based policy and planning documents, and represents a collaborative effort to lay the groundwork for addressing critical health promotion and prevention issues within the NEPHU catchment. Through coordinated efforts and shared learnings, NEPHU aims to work with partners to make a significant impact on improving the well-being of the population and fostering collaboration and networks among stakeholders.
